Every agent knows the first real conversation with a new lead can go sideways. You ask the same questions. The lead can’t remember their doctor’s name. Someone forgets their prescriptions. You end up with a second call just to fill in the blanks.

Good news. The Medicare Branning Bundle and the Health Insurance (ACA) Branning Bundle now come with insurance intake forms. A lead fills one out on their own time, and you walk into the call with the answers already in hand. (New to bundles? Here is how to use Branning Bundles on your existing website.)

Here is what the forms are, what they ask, and how to start using them today.

What’s New With Insurance Intake Forms in Agent CRM

Until now, these two bundles came with a funnel form that captured the basics on the funnel. That form is still there and still works.

Now each bundle also includes a full intake form. These forms are not on the funnel yet. Think of them as the next step after a lead raises their hand. You send the form, they answer your questions, and you get ready for a better conversation.

The forms are also smart. Based on how a lead answers, more questions show up. Someone with no prescriptions never sees the prescription follow-ups. Someone who has a planned surgery gets asked to describe it. That keeps the form short for people with simple needs and thorough for people who need more.

The Medicare Intake Form

The Medicare intake form has 24 fields across 7 sections:

πŸ“‹ Contact Information: name, phone, email, date of birth, and postal code
πŸ₯ Medicare Enrollment: Part A, Part B, and the effective dates
πŸ“„ Current Coverage: Medicare Advantage, Medigap, Part D, carrier, plan name, and any Medicaid or Extra Help
πŸ‘©β€βš•οΈ Doctors and Hospital: the doctors they want to keep and their preferred hospital or health system
πŸ’Š Prescriptions: what they take and their preferred pharmacy
πŸ—“οΈ Upcoming Care: any surgeries or procedures planned in the next 12 months
πŸ“ž Follow-Up: the best days and times to reach them

Enrollment season is the best time to put a Medicare intake form to work. See Medicare AEP 2026: A 9-Step System for Medicare Agents for the full plan.

The Health Insurance (ACA) Intake Form

The health insurance intake form has 28 fields across 7 sections:

πŸ“‹ Contact Information: name, phone, email, date of birth, and postal code
🏠 Household and Income: household size, estimated income for next year, spouse or dependents, and tobacco use
πŸ’Ό Employment: current work status and whether an employer offers coverage
πŸ“„ Current Coverage: what they have today, the carrier, and when it ends
πŸ”„ Reason for Coverage: lost coverage, new job, new baby, marriage, a move, open enrollment, or a plan that costs too much
πŸ‘©β€βš•οΈ Doctors and Prescriptions: doctors to keep, medications, and preferred pharmacy
πŸ’΅ Budget and Follow-Up: a comfortable monthly premium and the best days and times to call

How to Set Up Your Insurance Intake Forms in 4 Steps

It takes a few minutes. Do it once, and it is ready every time you need it.

  1. Preview the form. Open the intake form in your Medicare or Health Insurance bundle and look it over.
  2. Add the link to your Account Customizer. Paste the intake form link in the same place you added your Medicare SOA link, your calendar link, or your policy update link. This is what lets Agent CRM put the right link in your emails and texts.
  3. Publish the workflow. Open the red folders inside your bundle and find the workflow named Intake Form (Medicare has one, and Health Insurance has one). It is set to draft on purpose. We did not turn it on for everyone because adding the tag sends the form right away, and it needs your link first. Once your link is in, hit publish.
  4. Tag a contact. Add the intake form tag to any contact, and Agent CRM sends them an email and a text with the form. That’s it.

One thing to remember: if you tag a contact before the link is in your Account Customizer and the workflow is published, the message will not go out the way you want. Do steps 2 and 3 first.

Because the form goes out by text, make sure your number is ready: Insurance Agent A2P: How to Know Your Number Is Ready to Text. And for more on working leads after the first touch, read What to Do With Aged Insurance Leads.

Want Your Intake Form on the Funnel Instead?

You can swap the intake form in for the funnel form if you want leads to answer more questions up front. Some agents like the short form for cold traffic and the full intake form for warm leads. Try both and see what fits how you work.

Why Insurance Intake Forms Help You Win More Conversations

πŸ“ž Shorter first calls, because the basics are already answered
🧠 Better recommendations, because you know their doctors, drugs, and budget
⏰ Fewer back-and-forth messages to chase missing details
πŸ™‚ A better first impression, because the lead feels listened to before you ever talk

When a lead has already told you what matters to them, your time on the phone goes to helping, not collecting.

“I’ve been a customer of Alex Branning/The Branning Group (TBG) for almost a year now. I am a health insurance agent, and one thing I’ve learned is the absolute necessity for my business is to get high intent leads, and when I get them, I need to keep them engaged and informed in order to be ready to meet their needs when they are ready to buy. TBG has great tools (like their Agent CRM, Video Domino, and Pack My Calendar products) in order to accomplish these goals. I really appreciate the way TBG supports me; from the daily support calls, to the video training that comes with the products, TBG has everything I need thus far to successfully grow and manage my insurance business.”
